In response to a [[New York State Department of Health]] and AIDS Institute grant offering, six New York City HIV/AIDS providers collaborated to develop and launch Amida Care, previously known as Vida Care, in 2003. Amida Care has grown to develop a unique model of care to provide comprehensive health coverage and coordinated care to people living with and at higher risk for HIV/AIDS.

Amida Care's sponsor provider organizations offer specialized health services to communities throughout New York City. They are: Acacia Network, Brightpoint Health, Community Healthcare Network, Harlem United, Housing Works, St. Mary's Center, and Village Care.
Amida Care is a Medicaid managed care Special Needs Health Plan (SNP) in New York State that works closely with its members and surrounds them with a team of providers, social workers, health navigators, behavioral health specialists, and a designated medical home. <ref></ref>
In 2017, Amida Care expanded eligibility criteria to accept all transgender individuals, regardless of their HIV status. <ref></ref>
==Advocacy and Programs==
Amida Care advocates for policies and initiatives that support health and social justice for the populations it serves. Amida Care is a member of the [[Association for Community Affiliated Plans]] (ACAP), which represents publicly sponsored programs and health care providers who serve vulnerable populations.
Amida Care's President and CEO, Doug Wirth, served on Governor [[Andrew Cuomo]]'s Task Force to End the AIDS Epidemic in New York State by 2020, which developed New York State's Blueprint to End the AIDS Epidemic and reduce the number of new HIV infections to 750, from an estimated 3,000, by 2020. <ref></ref>
Amida Care addresses the social determinants of health that keep people out of care and prevent them from being healthy, such as unstable housing and limited employment opportunities. Amida Care has developed programs that provide workforce training and create peer health outreach jobs for its members.
Amida Care's Live Your Life wellness program includes events such as dance and nutrition classes, and is based on the belief that health is about more than doctor's visits and pills. Amida Care holds free monthly Live Your Life events throughout the five boroughs of New York City, where Amida Care members and their guests learn about ways to improve their health. <ref></ref>
Amida Care's holistic approach and its coordinated care model have yielded significant improvements in health outcomes: 76 percent of Amida Care's HIV-positive members are virally suppressed, resulting in cost savings of more than $115 million to New York State between 2008 and 2015 from reductions in avoidable hospitalizations and emergency room visits. <ref></ref>
